- Treatment: We will use your medical and dental informationto a physician or other healthcare provider involved in providing treatment with you. This includes providing medical and dentalinformationtopeopleoutside of East Columbus Oral Surgery Specialists who maybe involved in your care.
- Payment: We may use and disclose your medical and dental information to receive payment for our services from you, an insurance company or a third party. We may also tell your health plan about a treatment you are going to receive to obtain prior approval or to determine whether your plan will cover the treatment.
- For Health Care Operations: We may use and disclose your medical and dental information to operate East Columbus Oral Surgery Specialists. For example, we may use this information to review and improve the quality of care we provide, or the competence and qualifications of our professional staff.
